St. Martin Parish PreK Registration for the 2020-2021 school year
Please be prepared to upload the following documents:Birth Certificate (child) Social Security card (child) Driver's License (parent/guardian) Proof of Residency (parent/guardian) Income verification may also be required
You will be contacted through email once you have set up, completed and submitted the application. Please be sure to check your email associated with the registration system frequently.
If you attended registration at one of our schools, you will be contacted for information about returning missing documents as well as rescheduled testing times.

- 2020-2021 Affidavite of Residency Form - use this form if the electric/cable/water bill is not in the parent's name
- Statement of No Income Form - use this form if you are currently unemployed
- Irregular Income Form- use this form if you plan to submit 2019 tax forms as proof of income
- Statement of Exemption from Immunizations- use this form if you prefer not to have your child receive vaccinations
PLEASE NOTE ALL DOCUMENTS MUST BE PDF FILES. IF YOU UPLOAD JPEG OR IMAGES, WE CANNOT ACCESS THE FILES TO COMPLETE THE REGISTRATION PROCESS.
RETURNING DOCUMENTS FOR PREK REGISTRATION
Families that need to return documents to complete the registration process should have received a phone call with information about what documents need to be returned.
Missing documents may be brought to the Federal Programs Annex located at 625 Corporate Blvd. Breaux Bridge, LA 70517 on Tuesdays and Wednesdays from 8:30-3:45 starting on Tuesday, May 19th.
Please be aware that social distancing guidelines will be in place.
Once you arrive at the building there will be a sign with a phone number to call to be allowed into the building.
Only one person will be allowed to return documents.
Face masks/covering must be worn.

Our vision: St. Martin Parish is a community where all families have access and utilize high quality early childhood care and education services in which the whole child is educated.
Our mission: Through the use of accountable leadership, coordinated strategic planning and financial sustainability, the St. Martin Parish Early Childhood Network's mission is to provide and fill seats of high quality care and education to all eligible families in the parish by effective recruitment with a seamless process to understand eligibility so that all children benefit from high quality effective early childhood programs.
The goal of the St. Martin Parish Early Childhood Network is to provide families with quality child care and learning experiences for children from infant to five years old so they can be prepared to enter Kindergarten. St. Martin Parish School Board, CDI Agency (Head start) and Type III Child care centers are working together to increase the quality of all programs so that families have several choices for their child.
